Submission Instructions
Please mail the completed Application, Medical and Volunteer Forms along with a cheque for the membership fees to:
North Toronto Skating Club

If you choose to bring the forms and fees to the rink, please leave them with the Arena Manager's Office in the lobby.
APPLICATIONS THAT ARE RETURNED TO THE OFFICE WITHOUT THE MEMBERSHIP FEE WILL NOT BE PROCESSED. APPLICATIONS THAT ARE E-MAILED WILL BE NOT BE PROCESSED. APPLICATIONS THAT ARE MAILED TO/DROPPED OFF AT THE NORTH TORONTO OFFICE WITH FEE PAYMENT WILL BE ACCEPTED.
